Do I need Physical Therapy, and will it help?

Leslie will perform a detailed evaluation and explain what his findings and treatment plan. He will then discuss the course and duration of treatment as well as an estimate of the cost. If he determines the underlying cause to be outside of his scope of practice, he will let

you know, and recommend a suitable next step.

Do I need a referral from my doctor?

​

A referral is not required for the initial evaluation and treatment.  There are many rules which govern the requirements regarding referrals. If your circumstances warrant a referral, this will be

discussed with you.

What should I wear to my appointment?

​

Wear something comfortable and loose enough for your therapist to be able to get to the part of the body that is being treated.

Low, comfortable shoes are encouraged. 

​

Where do I go, and do I need to arrive early?

​

Our address and a map can be found on our contact page. Once you arrive at the L-shaped office complex, you will enter the door where the two arms of the L meet. Look for a large silver sculpture and my name at the top of the door. 

Once inside the door, go down the ramp to your right and you’ll see our waiting area slightly to the left. Your intake paperwork will be on a blue clipboard. Please arrive 10 minutes early to fill out paperwork.

Do you have a payment plan?

​

We have partnered with Care Credit, a trusted entity that offers interest-free credit for medical expenses. Those who sign up for Care Credit have the option to make minimum monthly payments over a

6-month period.

No interest is charged if payments are made in full and on time.

​

Note:  The full interest will be charged to your account if these conditions are not met.

​

What if there’s not an opening that fits with my schedule?

 

Call or leave a text message at our office and we will work to fit you in.

Do you take insurance?

​

We operate on a fee for service basis and do not take insurance.  However, we can provide you with the necessary information if you want to file for reimbursement with your insurance company.

​

 NOTE: WE DO NOT HAVE A CONTRACT WITH MEDICARE/MEDICAID AND YOU WILL NOT

BE ABLE TO FILE FOR REIMBURSEMENT WITH THEM.
